Supported housing for older people
Amber Valley Housing provides support services for older residents and people we know may be vulnerable. Our aim is to provide as much or as little support as they need to live happy, independent lives.
Amber Valley Housing works with other care and support agencies to ensure that:
- Tenants, are safe and secure in their homes;
- Emergency shopping is carried out for essential items;
- Social events are facilitated;
- Liaison takes place with families and carers;
- Advice is provided about housing and other topics.
Our Neighbourhood Support Co-ordinators will support residents by:
- Promoting independence, ensuring privacy, dignity, choice, respect and participation;
- Completing and agreeing support plans for all residents that need support;
- Carrying out visits as agreed in the support plan;
- Reviewing support plans every six months;
- Making sure that social activities are available.
You can help us by:
- Helping your support service co-ordinator to complete your support plan;
- Telling us if your circumstances have changed;
- Letting us know when you are going to be away from home;
- Letting us know if you begin to receive help and support from others, such as social services.
